(2016) and I would like to compliment the authors for a thorough study. It shows that preoperative start of thromboprophylaxis with low-molecular-weight heparin(LMWH) decreases 6-month mortality in femoral neck fracture patients receiving hemiarthroplasty. In a clinical setting, LMWH is a common chemoprophylaxis for venous thromboembolism (VTE) prevention. From the article, yet no VTE occurrence is stated. VTE is a major problem after hip fracture. Development of VTE is anticipated in the interval between the time of fracture and surgery since hip fracture patients cannot move the injured extremity during this period, while patients undergoing elective total joint arthroplasty are mobile. Preoperative VTE is known to occur in 3–62% of hip fracture patients (Salzman and Harris 1976, Zahn et al. 1999, Song et al. 2016).Yet no preoperative VTE is documented at the time of admission in the study. VTE is also a signifi cant risk factor for mortality. The mortality risks for patients with VTE were markedly higher during the fi rst year, especially within the fi rst 30 days after VTE diagnosis (Søgaard et al. 2014). Still, no association analysis was made between VTE and mortality in the study. Failure to bring VTE into the equation makes the association between thromboprophylaxis strategy and mortality hard to interpret in clinical practice. I would appreciate the authors’ thoughts on this.

Venous thromboembolism following major orthopedic surgery: what is the risk after discharge?

Guidelines recommend thromboprophylaxis for at least 10 days to prevent venous thromboembolism in patients undergoing high-risk orthopedic surgery, such as total hip arthroplasty (THA) or total knee arthroplasty (TKA). Furthermore, the recently updated ACCP guidelines also recommend extending the duration of thromboprophylaxis for 28 to 35 days following THA or hip fracture surgery as the risk ...

Does preoperative mechanical prophylaxis have additional effectiveness in preventing postoperative venous thromboembolism in elderly patients with hip fracture?—Retrospective case-control study

BACKGROUND Elderly patients undergoing hip fracture surgery (HFS) are at increased risk of postoperative venous thromboembolism (VTE). To reduce this risk, combined postoperative mechanical and chemical thromboprophylaxis has been routinely performed after HFS in these patients. This retrospective case-control study was conducted to evaluate the additional effectiveness of preoperative mechanic...

The compliance of thromboprophylaxis affects the risk of venous thromboembolism in patients undergoing hip fracture surgery

OBJECTIVE Venous thromboembolism (VTE) is major problem after hip fracture surgery with substantial morbidity and mortality. This study aimed to assess the postoperative compliance of thromboprophylaxis in elderly patients undergoing hip fracture surgery and to confirm the correlation between compliance and VTE risk. METHODS This retrospective cohort study included consecutive elderly hip fra...

Retrospective analysis of adherence to thromboprophylaxis after orthopedic surgery in a community hospital.

Venous thromboembolism is a well-known complication of total knee replacement, total hip replacement, and hip fracture repair. Venous thromboembolism, which includes deep vein thrombosis and pulmonary embolism, can cause significant morbidity and mortality. In addition, treatment of this condition can result in substantial costs to the health care system. Patients who have undergone orthopedic ...

Symptomatic venous thromboembolism following a hip fracture

BACKGROUND AND PURPOSE Venous thromboembolism (VTE) remains a substantial cause of morbidity and mortality following hip fracture. Previous work has not identified any risk factors associated with the type of hip fracture. We report the incidence of and risk factors for development of symptomatic VTE in patients following a hip fracture. PATIENTS AND METHODS In this prospective study, we coll...
